You will have unlimited use of the Database. Initial contact may be via email or by phone. However, It is recommended you schedule an In-Person Interview. What Level of Care Is Needed?
Caregiver/Homemaker Non-medical in
home care and assistance for seniors and
disabled adults. Assist clients with errands,
shopping, routine housekeeping including,
but not limited to vacuuming, dusting,
sweeping or mopping floors, doing dishes,
laundry, cleaning bathrooms and meal
preparation. Flexible work schedules.
Personal Care Assistant (PCA) Perform
personal care activities contained in a
written assignment by a Nurse Supervisor
which includes but is not limited to,
assisting the patient with activities of daily
living, such as ambulation, eating, dressing,
personal hygiene, and meal preparation.
Perform light housekeeping tasks that will
facilitate the patient’s self-care at home.
These tasks include, but are not limited to
vacuuming, dusting, sweeping or mopping
floors, doing dishes, laundry, cleaning
bathrooms and meal preparation.
Responsible for documentation of service
provided, using the Client Care Record.
Requirements: High school graduate or
GED, and current PCI or PCII Certification.
Certified Nursing Assistant (CNA)
Perform nurse aide duties, not limited to:
personal care of the patient activities, range
of motion exercises, vital signs
(temperature, pulse rate, respiration rate, and
blood pressure).Provide companionship and
personal care to convalescing, elderly, and
disabled individuals in private homes,
hospitals, and nursing homes. Other duties
may include light housekeeping, meal
preparation, errands, assistance with ADL'S,
(bathing, dressing, or hygiene, toileting,
feeding, transferring and ambulation) and
provide medication reminders. Responsible
for documentation of service(s) provided,
using the Client Care Record.
Requirements: High school graduate or
GED, and current Certified Nurse Assistant
(CNA) Certification.
LPN Perform initial assessments for both
Medicaid and Private Duty clients. Conduct
supervisory visits as needed. Provide
training to CNA's and Companion/Sitters
on an ongoing basis. Notify Personal Care
Physician/Agency of any changes in client's
overall condition, ensure safety of home, and
to ensure client's satisfaction with the
services being provided.
Nurse Practitioner Responsible for
providing quality health care services,
normally in a clinic environ. The position's
emphasis is on patient care in the inpatient
and outpatient setting, and staff education.
In addition to the primary emphasis on care
of the patient the NP's duties will also
include, but are not limited to, providing
patient assessments through patient exams,
patient medical history, interpretation of
required diagnostic procedures, and
developing/implementing treatment and
management plans of the outpatients under
the direction of a Medical Director or
Physician. This position requires working
closely with physicians, nurses, and other
staff. The NP shall meet or exceed current
recognized national standards established by
the Joint Commission of Health Care
Organizations and State regulatory agencies.
Registered Nurse
Direct responsibilities include performing
medication visits and initial assessments for
new clients, supervisory observation visits,
and assessment updates. Good
communication skills are required.
Communicate client needs, changes in client
status to Personal Care Physician/Agency &
Family Member(s) Maintain & Supervise
Client Medical Records and paperwork in
compliance with state regulations.
Organizational skills are a must.
SUCCESS TIPS for hiring the RIGHT Private Care Provider.
1. Invest an hour or two phone-shopping your local/area In-Home Care Agencies for Hourly/Daily rates appropriate for the specific In-
Home services needed by your family member. It is fair for you to know that Agency rates are usually double what the Care Provider
assigned to perform the duties will receive in wages. In addition to the hourly pay rate(s), you help cover the costs for Payroll Taxes,
Administration, Insurance, Overhead and Profit.
With that in mind, you should easily be able to negotiate a very reasonable hourly/daily/weekly rate with your selected Private Care
Provider for 30-50% LESS than Agency rates.
The BEST part about this method is, after a thorough interview, reference verification and selection process - YOU (not the Agency)
can invite the BEST person into your home to provide the level of Personal Care and Attention your family member deserves. Also,
most agencies frequently "rotate" their staff assignments. With a Private Care Provider you'll spend fewer hours in (re)orientation, as
to the duties and schedule of care.
2. The "Employer/Employee" dilemma that presents a major challenge to many Family Caregivers in search of a Private/In-Home
Caregiver can be resolved by offering an agreement for services rendered as a "Non-Employee" Independent Contractor. "Employers"
are required to withhold and report Income Tax & Social Security from wages paid to an "Employee", then pay a matching amount.
So, instead of "Hiring an Employee", you can take the initiative to let the Caregiver know you are willing to accept their services as a
"Non-Employee" (or Sub-Contractor) and furnish them with a 1099 Miscellaneous Income Wage statement. Your CPA will provide
you with the completed form(s) by the usual January 25th deadline for the previous year's taxable earnings.
NOTE: The IRS "W-9" form that requires their Social Security # and signature authorizes you to report payments made by you as
"Non-Employee" wages. This form can be obtained at: www.irs.gov/fw9.pdf The person/family for whom services are provided
simply files the 1099 with their Income Tax return and send the worker's copy to them to use as a wage statement when filing their
return. This simple solution totally eliminates the burden of Client tax payment responsibility and liability, and the wages paid may be
tax deductible, subject to the maximum for Health-related expenses. You should Consult your CPA for clarification.
3. Let your prospective Care Provider know you expect Full & Complete Disclosure regarding their credentials. Here are some "items
that should never be overlooked or avoided.
- Insist on no less than 3-5 verifiable Family Caregiver job references to whom they were assigned. Be sure to ask for phone
numbers. They will have them. If not, BEWARE.
- BE SURE to call ALL Personal References. Invite and encourage frank opinions regarding the Prospective Care Provider's
Work Ethic, Moral Habits, Dependability and Reliability.
- ASK FOR and expect them to furnish you with proper documentation. A 10yr. Department of Motor Vehicle Report, current
Criminal Background Check, Retail Credit Report, current Training/Re-certification (including First Aid & CPR if applicable),
proof of current Immunization & Tuberculin skin test results (PPD) are required for ANY Agency employment. You should
require nothing less.
- Feel free to inquire about physical limitations or medical conditions of the prospect. You'll need to know if they may need to
depend on you for help. Your caregiver needs will require them to be able to perform and function as needed.
- AVOID hiring Illegal Aliens, as severe penalties may result. They may offer to work for less today, but will cost you more in
the long run.
- Recruiting a Private Care Provider is not necessarily something everyone can do. It may require more effort than what a
Family Caregiver with a serious medical condition medical condition can accomplish. If this is the case, and you have no other
relative or friend to help handle the verifications and interview process - do not attempt it. That is when the additional
Agency expense is justified and necessary.
4. Another concern confronting Family Caregivers is the matter of Injury/Liability, while performing services. It is advisable to have
an understanding with the Care Provider by including a statement in your written agreement with them stating they: "...Agree to hold
the Payor/Family/Client fully harmless from any and all liability resulting from an injury or loss sustained, in the course of providing
services to the Client/Family Member, or while on your property". As for any Liability issue regarding injury or loss suffered by a
Care Provider, most Homeowner Insurance policies do provide a limited Medical benefit, and a claim for injuries sustained by a non-
occupant, or visitor (including Care Providers) would be covered.
5. You should also make a WRITTEN, Signed & Dated agreement that includes: a. Work Schedule, b. Duties/Responsibilities, c. Con-
fidentiality acknowledgement, and d. Compensation/Pay rate. You can view a Sample Agreement from a link here on the Family Info or
