Care Providers have personally sworn that Statements of Information he/she has submitted to our
database is Accurate and Current, to the best of their knowledge and belief. To retain a current listing
in our database, Care Providers are required to update and certify the accuracy of all information every
90 days. An updated SLED (background) investigation is required every 12 months.
Furthermore, ElderCare Service Providers assumes no liability or responsibility, directly or indirectly for
any claim of property loss or personal injury that may arise, as a result of the employment or selection
of any Care Provider.
Senior-Assist CONNECT is a Listing & Referral service only. No condition of employment exists between
the Care Providers and ElderCare Service Providers.
